The Mom NP

Effective date: October 22, 2026  |  Last updated: October 22, 2026

The Mom NP (“The Mom NP,” “we,” “us,” or “our”) respects your privacy. This Privacy Policy explains what information we collect through npformoms.com (the “Website”), how we use and protect it, who we share it with, and the choices you have. Please read it together with our Terms & Conditions.

1. Scope of This Policy, and How It Relates to Your Health Information

This Policy applies to information we collect from visitors to the Website, including people who contact us, request an appointment, subscribe to updates, or interact with us online.

Patient health information is handled separately. If you become a patient, the health information we create or receive about you in providing care (“protected health information” or “PHI”) is governed by the Health Insurance Portability and Accountability Act (“HIPAA”), Florida law, and our Notice of Privacy Practices, which is available at our office and on request. If this Policy and our Notice of Privacy Practices differ for PHI, the Notice of Privacy Practices controls. Information you submit through the public pages of the Website before you are a patient is generally not PHI, but we still treat it with care.

2. Please Do Not Send Medical Details Through the Website

Website contact forms, standard email, and standard text messages are not secure channels for sensitive health information. Please do not include diagnoses, symptoms, medications, lab results, insurance details, or other medical information in a form, email, or text message. To discuss your care, call our office at (850) 990-0636 or wait for your visit. If you choose to send health information through an unsecured channel, you do so at your own risk.

3. Information We Collect

Information you provide to us
  • Contact and appointment requests: name, email address, phone number, preferred contact method, and the content of your message.
  • Communications: records of emails, calls, text messages, and form submissions you send us and our replies.
  • Marketing preferences: your choice to receive or stop receiving newsletters, offers, or text messages.
Information collected automatically

When you visit the Website, we and our service providers automatically collect technical and usage information, such as IP address, device and browser type, operating system, referring pages, pages viewed, links clicked, date and time of visits, and approximate location derived from your IP address. We collect this through cookies, log files, and similar technologies (see Section 6).

Sensitive information

We do not intend to collect sensitive personal information through the public Website. If you voluntarily include health information in a message, we will use it only to respond to you and to provide or arrange care, and we will not use it for advertising.

Information from other sources

We may receive information about you from service providers that help us operate the Website and from review or business-listing platforms such as Google.

4. How We Use Information

  • To respond to your questions and schedule or manage appointments and consultations;
  • To provide, operate, secure, and improve the Website and our services;
  • To send administrative messages, such as appointment confirmations and reminders;
  • To send marketing communications, where permitted by law and with your consent where required;
  • To understand how visitors use the Website and to improve its content and performance;
  • To detect and prevent fraud, abuse, spam, and security incidents;
  • To comply with legal, regulatory, and professional obligations, and to establish or defend legal claims.

5. How We Share Information

We do not sell your personal information, and we do not sell sensitive personal data. We share information only as described here:

  • Service providers. Companies that help us run the Website and our business, such as website hosting and security, spam protection, email and form delivery, scheduling, communications, analytics, and review-display providers. They may use your information only to perform services for us. Where a provider handles PHI for us, we enter into a business associate agreement as HIPAA requires.
  • Professional and legal advisers, such as attorneys, accountants, and insurers, when needed.
  • Legal and safety reasons. To comply with law, regulation, subpoena, or court order, or to protect the rights, property, or safety of our patients, visitors, staff, or the public.
  • Business transfers. If The Mom NP is involved in a merger, acquisition, or sale of assets, information may be transferred as part of that transaction, subject to this Policy and applicable law.
  • With your direction or consent.

We do not use or share PHI for marketing without your written authorization, and we do not sell PHI.

6. Cookies, Analytics, and Tracking Technologies

Cookies are small files placed on your device. We and our providers use cookies and similar tools to keep the Website working, protect it against spam and abuse, measure traffic, and display reviews. These tools may collect information such as your IP address, device identifiers, and the pages you visit.

  • Essential tools support security, spam protection (such as Google reCAPTCHA), page loading, and basic functions.
  • Analytics. We use Google Analytics and Google Tag Manager to understand aggregate usage of the Website, such as which pages are viewed. We do not use these tools to collect the content of messages or form entries, and we do not use session-replay software.
  • Embedded third-party content, such as maps and Google reviews widgets, may set its own cookies when you view it.

Your controls. You can block or delete cookies in your browser settings, although parts of the Website may not work as intended. You can also opt out of Google Analytics using Google’s browser add-on or your browser’s privacy settings. We honor opt-out preference signals, such as Global Privacy Control, where applicable law requires. We aim to keep advertising and analytics tools off pages where you submit health-related information. This Policy and our use of cookies do not authorize the disclosure of PHI to any tracking vendor.

7. Email and Text Message Communications

Email. Marketing emails identify us as the sender, include our postal address, and include an unsubscribe link. We honor unsubscribe requests within 10 business days. We may still send you non-marketing messages, such as appointment confirmations.

Text messages. We send marketing text messages only with your prior express written consent, which you may give by an affirmative act such as checking a box, and which is never a condition of purchasing any service. Appointment reminders and similar messages are sent according to the preferences you give us. Message and data rates may apply, and message frequency varies. You can opt out at any time by replying STOP, and reply HELP for help. We do not sell or share mobile phone numbers or text-message opt-in data with third parties for their own marketing. Because standard texting is not secure, please do not send health details by text.

8. Your Choices and Rights

You may contact us at any time to ask what personal information we hold about you from the Website, to correct it, to request that we delete it (subject to legal and record-keeping requirements), or to withdraw consent to marketing communications. Email Lisa@NPforMoms.com, call (850) 990-0636, or write to us at the address in Section 14. We may need to verify your identity before responding, and we will respond within a reasonable time as required by applicable law.

HIPAA rights. As a patient, you have separate rights over your PHI, including to access it, request amendment, request restrictions, receive an accounting of disclosures, and request confidential communications. These are described in our Notice of Privacy Practices.

9. Data Retention

We keep personal information for as long as needed for the purposes in this Policy, to meet legal and professional record-keeping requirements, to resolve disputes, and to enforce our agreements. Medical records are kept for the periods required by Florida and federal law. When information is no longer needed, we securely delete or destroy it.

10. Security and Data Breaches

We use administrative, technical, and physical safeguards designed to protect personal information, such as access controls, encryption in transit through HTTPS, and security monitoring. We require our service providers to protect the information they handle for us. No method of transmission over the internet or electronic storage is completely secure, so we cannot guarantee absolute security. If a breach affects your information, we will notify you, and regulators where required, as required by the Florida Information Protection Act, HIPAA, and other applicable law.

11. Third-Party Websites and Services

The Website may link to or embed third-party sites and services, including social media, map, scheduling, and review platforms. We do not control them and are not responsible for their privacy practices. Please review their policies before sharing information with them.

12. Children’s Privacy

The Website is intended for adults and is not directed to children under 13. We do not knowingly collect personal information from children under 13. If you believe a child has provided us with personal information, contact us and we will delete it. Treatment of minors is handled in the office with a parent or legal guardian.

13. Changes to This Policy

We may update this Policy from time to time to reflect changes in our practices, technology, or the law. We will post the updated version on this page and revise the “Last updated” date. If a change is material, we will provide additional notice where required.
